Summit Student Resources

 

Veterinary Students:  Please fill out the AVMA PLIT Student Professional Liability Application, as well as the NC Veterinary Medical Board 4th Year Student Application prior to your time with us. 4th-year veterinary students at NCSU that are doing an official externship with us are already covered by the school’s insurance but we do ask that you send us a copy of proof of insurance before your arrival.

Undergraduate Students:  Please fill out the Student Volunteer Waiver and Emergency Contact Form prior to your arrival.  

 

It is best that all paperwork be submitted to Summit Equine 30 days prior to your rotation start date.
